But that "someone" is his parents. They are the ones responsible for that decision, AND the consequences of that decision.

I can't speak for the US system but when such a case arises in Scotland there would be a Curator Ad Litem appointed whose sole brief is to look after the interest of the child taking cognisance of the religious background of the child and their wishes in the matter in so far as the child can express them. A 15 yr old who can clearly express their wishes in the matter will have more attention paid to them than a young child. The wishes of the parents have to be clearly and properly weighed against the needs of the child and the medical recommendations.
This has led to some parents wishes for the support of life of a baby not being terminated to be overturned and sometimes for treatment to be gibven against parents wishes. Here the defence of religious objection is only one strand.
